  • How one NHS anaesthetist is fighting international medical research fraud

    https://theconversation.com/how-one-...search%20fraud

Carlisle’s method does not definitely say whether a trial report is fraudulent. It’s a screening method that suggests that some trial reports need to be examined more thoroughly to check whether anything untoward is going on. There could, sometimes, be innocent explanations for the unusual patterns of data that the method detects.

                https://bmcmusculoskeletdisord.biome...891-019-2796-5

Evidence of low certainty suggests that eccentric exercise may provide a small but likely not clinically important reduction in pain compared with other types of exercise in patients with subacromial impingement syndrome. It is uncertain whether eccentric exercise improves function more than other types of exercise (very low certainty of evidence). Methodological limitations of existing studies make these findings susceptible to change in the future.

                • Cervical proprioception and its relationship with neck pain intensity in subjects with cervical spondylosis

                  https://bmcmusculoskeletdisord.biome...891-019-2846-z

To date, to our knowledge, this is the first study of its kind that has evaluated and shown a significant positive correlation between neck pain intensity and position sense in subjects with CS. Pain models that were experimentally induced showed a positive influence between pain and proprioception [31]. The substances that are chemically mediated during the pain response might alter free nerve ending discharges due to sensitization and produce abnormal pain afferents (gamma-motor neuron and muscle spindle), thus impairing kinesthetic input [32]. Contrary to our study, Lee et al.’s study did not show any association between neck pain intensity and cervical proprioceptive sense in chronic neck pain subjects; rather, they noted that a higher pain frequency was positively associated with increased JPEs [6]. Our study results cannot be compared with et al. study as the sample characteristics and cervical proprioception testing methodological considerations are different from the current study. Further studies are required to see the association between neck pain frequency, duration, and intensity and proprioception in a larger sample of CS subjects.

                          https://bmcmusculoskeletdisord.biome...891-019-2843-2

Trunk muscle dysfunction is often regarded as a key feature of non-specific chronic low back pain (NSCLBP) despite being poorly understood and variable with increases, decreases and no change in muscle activity reported. Differences in thoraco-lumbar kinematics have been observed in motor control impairment NSCLBP subgroups (Flexion Pattern, Active Extension Pattern) during static postures and dynamic activities. However, potential differences in muscle activity during functional tasks has not been established in these subgroups to date.

                              https://bmcmusculoskeletdisord.biome...891-019-2889-1

Chronic musculoskeletal pain affects more than 20% of the population, and the prevalence is increasing, causing suffering, loss of quality of life, disability, and an enormous expenditure on healthcare resources. The most common location for chronic pain is the spine. Many of the treatments used are mainly passive (pharmacological and invasive) and poor outcomes. The treatments currently applied in the public health system do not comply with the recommendations of the main clinical practice guidelines, which suggest the use of educational measures and physical exercise as the first-line treatment.
