I've had close looks at this text before, and didn't like how it focused on the spinal column as if treating the spinal column were the only way to modulate pain in a human being..
However, in googling around for this and that I found this online chapter (http://books.google.com/books?hl=en&lr=&id=SwhoZYtgVd8C&oi=fnd&pg=PA155&dq=Moog+M,+Quintner+J,+Hall+T+and+Zusman+M+(2002)&ots=7rKxqJkRj0&sig=hLKJTJv_bWJfToiCS_6rL5deoXU), which may be of some use to someone. It starts at p. 155.
Here is another version (http://books.google.com/books?hl=en&lr=&id=SwhoZYtgVd8C&oi=fnd&pg=PA413&dq=Moog+M,+Quintner+J,+Hall+T+and+Zusman+M+(2002)&ots=7rKxqJkWe2&sig=Wht3UsGe1cQzhw7qVidgNui5XSc), starting at p. 413.
