Immobilisation in plaster for 2 weeks=(why???)--> ischaemic effect

Bilateral venous and arterial insufficiency (premorbid?)--> ischaemia++

Ischaemia in tissues, especially nerves=pain.

Stiffness (for no good reason)-->abnormal gait--> increased stress.

As they now know that even for #s of scaphoid should be only bandaged and NOT plastered (according to one study, the risk of avascular necrosis is higher if plaster is applied), this man was poorly managed.

Any more information?

As I said, the picture was too bright and simple!
But I had a RX of his R leg and it shown only the leg and not the knee but my later examination found an exquisite point on peroneus near the travel of external sciatic nerve. A new RX determined a fracture!

Sometimes we fail because the clues show an idyllic picture that make us think that it cannot be another problem!
