Hi all those are some ideas i have read in one article describing how the physio could be expert :

Deliberates Judgement

* deliberates on ends and means ,critical disposition,taking nothing for granted ,break out of habitual ways of seeing ,question what is accepted .

* Makes meaning of theory and practice ,examines critically own moral values ,using practical reasoning and practical wisdom .a holistic approach.


So something s of what i have written i can noy undersatnd as ;

Hi emad
I will try and answer your questions, and I am sure others will as well.

Ends and means:

If we choose methods (the means) we hope to achieve goals (the ends).

So if we teach patients about the reasons for the pain, so they understand why it is there, and what they can do about it, they are calmer and worry less. We give them the skills so when they leave your rooms, they can manage their pain better, and know it will not 'harm' them.
A physiotherapist might also do manual therapy to help them to relax more, and it might send good messages to the brain.


Critical disposition:

A good physio looks closely at what might be done, decides if that is best for what has been found at assessment, and does not have to follow 'The Rules'. We criticise what we read, the research done may not be accurate or valid; we may criticise what we have been taught that is now out of date.

A patient may be complaining of knee pain, and the doctor has said it is patellarfemoral pain. We may agree, we may not. We may find another reason , and treat that because it 'fits' better.

A study may find that physiotherapy is not helpful for acute back pain.
if we criticise, look more closely, we find all that was done was ultrasound and exercises, some of which made the pain worse.

We know we can do a lot with acute pain; and every patient is different. Each one is different because everyone reacts differently to pain. Home life may be stressful++ - and so on.

Unless we are careful, we are just technicians, following recipes.
